and another one for the retrospective data analysis to gather enough evidence to ensure the achievement of the impact.

Spain

Cardiovascular Disease
This study intends to stratify more precisely patients with heart failure based on their clinical situation, along with other modifiable factors such as environmental factors, lifestyle habits, and therapeutic adherence. The main objective is to improve the level of cardiovascular health of the population.

Switzerland

Type 2 diabetes
This pilot will be focused on type 2 diabetes. With the implementation of this study, it is intended to more precisely stratify patients with type 2 diabetes based on their clinical status along with modifiable factors such as environmental factors, lifestyle habits, and therapeutic adherence. An additional focus is on stress and sleep behaviour changes, which are rarely assessed in clinical practice but can exacerbate unhealthy lifestyles.

Italy

Diabetes and cardiovascular diseases
In the pilot, approx. 1,000 people diagnosed with diabetes will be involved and cared for in the Endocrinology Unit, evaluated and screened for metabolic, anthropometric, clinical and hormonal variables which will be used to estimate the risk of cardiovascular and renal complications. The main objectives will be to define the risk of these people with diabetes to develop a diabetes complication (CV disease, renal disease) and to detect the risk of patients with both diabetes and CVD at baseline to face further disease progression.
